Chasing the Virus

Your two posts above make medical sense neither the “virus” or the “vaccines”” are behaving typically.

That is amazing when one of the most common respiratory virus behaves the same way and the vaccine(s) used to combat it has the same level of effectiveness.

The longer this pandemic progresses the more the “science” is supporting my assessment that COVID-19 will become one of the common respiratory viruses (diseases). These viruses have the common:-

1) the recovery rate eventually being high even without a vaccine.

2) They still takes lives worldwide by the hundred of thousands. This is usually from those that are immuno-compromised and were not vaccinated. This is in comparison to the hundreds of millions that recover.

3) A “one and done” vaccine does not exist. This is due to the fact that these viruses mutate rapidly (in comparison to most viruses). A (major) virus mutation does not necessarily make it significantly more debilitating \ fatal. It may make it more evasive to the immune system and in this way MAY cause more deaths INITIALLY. The immune system has to (re)recognize these mutations as a threat and mount a response. This is how the immune system functions. Through our lives it recognizes MOST pathogens and mounts a response to MOST of them. Vaccinations help reduce the length of time to re-recognise the virus\mutations and for mankind to develop a collective immunity.

My question is why is mankind “reinventing the wheel” when we have been exposed to viruses that behave the same as COVID-19 and are still commonly spread. We have not been able to eliminate these viruses and these viruses have not eliminated mankind. My observation is we have been “blinded” by non scientific fake news, the politicizing of the public health disease prevention measures and the utter nonsense from anti-vaxxers. Also, some of the mixed messaging from the scientific and medical community has only added to the uncertainty in combating this virus. What we must do is to logically assess this virus after a 20 month pandemic against the history of fighting similar viruses\diseases and let modern science help tame this pandemic in the shortest time possible.

As supported by the prevailing science and the data on COVID-19 this is my assessment on the pandemic

1) border closures, lockdowns & curfews will become useless to stop the spread in any meaningful way in the long term. The advent of the delta variant HAS and WILL make this abundantly clear. Those measures were used with the intention of being temporary and that the pandemic would have eventually ground to a halt. This is now wishful thinking. Far from coming to a halt, only more variants have arisen. What these measures may do is to provide a temporary reduction of cases. This is just too costly, unsustainable and ineffective for the purpose of ending this pandemic. More targeted measures are needed.

2) The first measure is to have a deep and rapid vaccination program. Barbados and countries the world over have gone through mass vaccination programs to viruses before (including to viruses that literally have been more crippling). We know from influenza that vaccines and boosters are devised yearly for particular influenza subtypes(variants). They have not been mandated as influenza now has a relatively high recovery rate unaided. My estimation is that countries should have the CAPACITY to immunize 60% – 70% of the population over 6 months for the next 1-2 years. This falls inline with estimates of the length of time particular vaccines are effective. Not strictly getting to this number does not mean failure as this virus has a high recovery rate to begin with . it will only determine how long we will stay in this pandemic. The current vaccines have been given “emergency use authorization” after going trough successful Phase 3 trials. Real world data is supporting the full authorization of these vaccines. This will strengthen the argument of mandating vaccines at least until the pandemic is brought under control. Vaccine mandates are far from unique. In my opinion when these vaccines are fully authorized, there must be a universal mandate of either to become vaccinated or be subjected to 7-14 day testing at least for the next 12-24 months. This mandate can occur when there is enough vaccines\boosters to vaccinate 60-70% of the population

3) Mass isolation \ quarantining will become impractical and basically unnecessary for THIS virus. In Barbados, the data is showing that the majority of cases are in tertiary isolation. Gov’t is now considering monitored home isolation. In my opinion this is essentially supervised sick leave. Doctors authorize SICK LEAVE with intention that people STAY AT HOME to recover on their own.
The danger these people with COVID pose to the rest of society is less if we continue to adopt the public health measures of sanitizing, temperature checks and mask wearing for all including the vaccinated. In my opinion this monitored isolation should only be for the unvaccinated. In theory, those vaccinated that develop Covid-19 should pose an even lesser threat when directed to be on “sick leave”

In certain environments e.g. in bars or restaurants, improper mask wearing occurs for prolonged period without the required social distancing This is an obvious, unwanted and now unacceptable gap in the protocols. The CMO has stated that sanitizing the work place after the discovery of a single COVID case is not obligatory. I would support the idea that this sanitizing be mandated by the Ministry of Health if such a business has been found to be the source of a significant cluster or has been the destination of several cases. The MOH should perform a specific “COVID-19 audit” of said businesses to modify operations to make them compliant to prevent future COVID-19 spread.

  • Instead of worrying too much about Covid it is better to concentrate on the problems of old age and how it affects health and fitness especially for the inactive. When you hit 50 your body starts losing strength and you need awareness to recognise health problems and issues and should formulate strategies to address them as they arise. Overdoing exercise can also cause injuries so you should adopt easier restorative practises. It’s all about Inner Management.

    … or they are going to bars en masse to drink a liquor.

    So logic suggests fathers and mothers are taking their children to bars and exposing them to the virus which causes so many young people to be testing positive!!

    We clearly need to separate children from their parents!!!!!!!!!!!!!!!!!!!!

    It is de ffing water coming into the home where families are locked down and confined by the GOB!!

    @ Crazy John…It was noted in Barbados earlier this year when there was a backlog in testing, that family spread is a reality. It is also a place where there is a gap in following the basic virus prevention protocols. This has been a reality with influenza and common cold spread. So what do we do? At home we can try to avoid prolonged contact with that family member as we try to do when they have the cold!!!. There is nothing wrong in enforcing the basic health protocols at home at least for the duration of the pandemic. We do not need to shun people with COVID-19 as though (or to the extent) as if they have leprosy or measles.

    What I am emphasizing is the need to “dial down the dread” associated with the virus and fight it with rational targeted strategies based on mankind history of fighting similar viruses. We have a pandemic of a respiratory virus that is new to man. It is a clearly a threat especially to those that are immunocompromised. Vaccinations can directly help some of those people, having the population vaccinated can help those persons indirectly by reducing the spread.
    Based on the science and data available I outlined steps that could be take for the next 12- 24 months that does not call for the unnecessary, irrational destruction of our economy and society.

    Of course, lockdowns and curfews can be a double edged sword. It can cut both ways.

You can test your hypothesis yourself by drinking water from different sources.     That might explain how the whole family of one BU contributor living up in the heights of St. Thomas, growing yams, minding their own business and not frequenting bars with their little children came down with the Asian flu in the 57/58 pandemic.

    I am pretty sure Bars would not be considered back then as the reason for that family cluster. People would not be so dumb back then.

    If the gap she lived in as a little girl was supplied by a standpipe, then chances are every family up her side was a family cluster back in 57/58 …. assuming they congregated at the standpipe at the same time to catch their water, gossip and cuss one another.

    Incidentally, zoning and attention to water quality came into effect in the 60’s.

    So maybe mankind did invent the wheel back then.

    So why is mankind today looking to reinvent the wheel?

    I think that your idea that we have to plan to live with Covid 19 long term is correct and so we need a strategy that is sustainable.

    I too think that we need more targeted measures but I don’t see mass vaccination of everyone as being very targeted. First, it’s not achievable and trying to coerce people into being vaccinated, apart from being immoral, is counter- productive. There is a general lack of trust among the population for politicians and the big pharma companies that are pushing mass vaccination. Someone asked me recently why they are giving us this vaccine for free when medication for diabetes and high blood pressure, which are also epidemics in Barbados and the Caribbean, is so expensive?

    The other problem that needs to be acknowledged is that the vaccines are not risk free. There have been thousands of reports of vaccine induced injuries and deaths in countries which keep these records and have them publicly available. Also, not everyone is at risk of hospitalisation and death from Covid infection. This is why a targeted approach is so necessary. I find it extremely frustrating that not only in Barbados, but across the region, after all these years of independence, we appear to lack even the basic research capability to analyse the patterns of Covid infection, hospitalisation and death in the Caribbean in order to provide public information that could guide an appropriate response for managing Covid 19 long term.

    Such a targeted approach needs to include prevention and treatment of those who become ill. With regard to prevention, we should be looking at identifying the factors which seem to lead to initial infection. These should include environmental and biological ones. When contact tracing is done and some contacts are found to have been infected while others are not, what data is collected on the differences between these individuals? Wouldn’t analysing the data from thousands of contacts begin to give us a more precise picture of the different risk levels for infection? Couldn’t a similar approach be pursued with regard to hospitalisation and death? On the basis of this level of detailed analysis, it should then be possible to determine the appropriate type of prevention for each individual. This might then involve offering vaccines to those who are realistically at risk of hospitalisation and death from Covid, while avoiding unnecessarily vaccinating people who are not likely to develop severe Covid or putting them at risk of vaccine injury.

    A targeted approach also needs to focus on treatment. The vast majority of people who become severely ill from Covid recover. We need to be pragmatic and explore all possibilities when it comes to treatment. There are numerous reports from doctors who have successfully treated patients, including through the use of ivermectin and other treatments that I mentioned in my original post. Why can’t we set up our own tests here in Barbados, following all the necessary ethical guidelines, to see if any of these treatment regimes work?

    Unfortunately, Covid 19 has exposed the underbelly of Barbados and the wider region and underlined that in the 21st century it’s a crime against the people not to have a modern functioning health care system free at the point of use. Covid may be in the spotlight now but long before Covid, people in the region have been dying unnecessarily due to a lack of access to proper health care. Hopefully the lesson we will learn from this experience is that this situation is no longer acceptable.

    that’s what people have been telling them from the very beginning, but their bullying style and holding job losses over people’s head like a weapon to meet a quota, to appease crooks got them push back…..education to the pros and cons of the vaccines was the intelligent approach, but the parliament have been short on intelligence and even less on common sense for 55 YEARS…so don’t expect any miracles now…

    Barbados has “free” health care at the point of delivery.

    Apart from that I think that your approach is the one we need to take. Covid is not going anywhere. I think some places are using early treatment for even those who appear asymptomatic because it has been discovered that there could still be residual effects from the infection.

    We have to learn to work around this virus.

  • All we have to do to get rid of COVID is clean up our act with respect to water.

    We know how to control immigration.

    COVID is in the water distribution system.

    It can be eradicated.

    We routinely effect the process our selves of clearing contaminants from our water.

    If there is a burst pipe in the area, in fixing it the maintenance crews disturb the rust inside the pipe and soil also gets into the pipe being repaired.

    The result is that for a while the water at the consumers’ (plural) tap is brown and discoloured.

    Depending on when the individual consumer (singular) turns on the tap the level of concentration may be high or low.

    All the consumer does is run the water for a while to clear the contaminants.

    It then returns clear and is used.

    But what if the contaminant leaves the water colourless as usual?

    We are none the wiser and consume it regardless.

    We drink it, we bathe in it, we water our plants or whatever.

    Depending on the concentration of the contaminant we can get a real heavy dose which overwhelms the immune system.

    That’s why those with strong immune systems are now getting infected.

    That’s why we see the numerous family clusters.

    We explain it by saying the Delta variant is highly contagious which it might very well be, but every variant will be evenmore highly contagious once it is in the water.

    The solution is pretty straightforward, same as what we do when there is a burst pipe but on a bigger scale.

    Targeted as in fighting the virus at the biological \ microscopic level in comparison to using lockdowns, curfews, mass isolation and border closures. As I stated I’m using how mankind has fought \ tamed similar viruses before. Note that not everyone takes influenza boosters yearly or do they the need to. My strategy does not call for compulsory vaccination but, for the purpose of ending the pandemic( health emergency) in the shortest time, I am calling for as many people to take the vaccines\boosters in the shortest time possible OR be tested. The concept of “diminishing returns” will come in as less people will have that mindset to be vaccinated after each round. The out come will be for more more major mutations to take root yearly. As I stated major mutations do not make them significantly more fatal, they may make them more contagious (as the Delta Variant is showing for Sars-COV2). If one has tested positive they go on sick leave from work and not allowed in enclosed crowded spaces. This is what SUPPOSED to happen when one has a contagious, respiratory virus. After the 1-2 years it could be a simple mandate of wearing masks in indoor spaces to reduce the threat of contagious respiratory viruses in general. The immune system will and has proven to do the rest. I am hoping that after 1-2 years that the stigma and the politicizing of fighting COVID-19 has been vastly eliminated

    You also state this distrust of “Big Pharma”, that vaccines are not “risk free” but yet state that ivermectin has had some success as though it is “risk free”. Also you want to compare the cost of a vaccine to fight a contagious virus to drugs that combat NCDs which may have a genetic and lifestyle origin. It is “immoral” to take a vaccine but more moral to take a drug? And nonsense about unnecessarily vaccinating people and risk of “vaccine injury”?. As Bajans would say you are like Poonka “ALL OVER THE PLACE!!!!!!” with your position. Let me clue you in that Big Pharma makes both vaccines, drugs and involved in hormone extraction\synthesis to fight NCDs like diabetes. They will make money regardless of which route is taken to fight the virus. They have the finances and infrastructure to develop both.
    There is a reason why “Big Pharma” and even individual countries(Russia, China, Cuba) resorted to vaccines than to drugs. They are far safer (not completely risk free) and in the case of the Sars-COV2 virus (that has a high recovery rate) they are guaranteed to be far more effective and far more easier to develop (depending on the vaccine type used). Vaccines give persons an acquired immunity by exposing them to part of the virus. This is far safer than having that acquired immunity from being exposed to a live replicating, debilitating virus. The “purpose” of the immune system is to acquire an immunity to most pathogens throughout the life of the person. For COVID 19 the vaccines are really for the immunocompromised. The worst that will happen is that the immune system is weak\depressed and will not take up this immunity so more boosters may be needed. This is the main reason for the “breakthrough” infections. For the more healthy individuals, vaccines help reduce the extent people become “Sars-COV2 factories”. They reduce the risk of developing symptoms\ complications from the virus. They help to reduce the amount of virions people expel hence it will control the spread.
    Drugs are resorted to when viable vaccines have not been developed or possible. Usually those viruses are the most devastating to the human body. Drugs slightly alter the individual’s biochemistry and in this way are less safe (in comparison). This why they take longer to be developed and approved. Hence, for me ivermectin & remdesivir has to go through specific Phase III trials for COVID-19 Anecdotal evidence is not enough. We know from other common drugs e.g. acetaminophen, ibuprofen & aspirin that severe side effects can occur if not used properly. The Phase III trial would give us the effective and correct dosage to use these drugs as a prophylactic for Sars-COV2 or in the treatment of COVID-19.
    BTW this is why vaccinations will always be cheaper than drugs to treat NCDs. It is easier to get rid of foreign pathogens using the immune system than to permanently treat\cure a chronic illness.

    Since we are dealing with a respiratory virus spread by droplets, the data is showing that the same environmental factors that promote the common cold and influenza are the same here. The difference is that Sars-COV2 and its variants has proven to be more resilient and more contagious. Since we are dealing with a virus, those that are immunocompromised have a weak immune system are at a higher risk. Since it takes a medical evaluation to determine how the state of your immune system, it is easiest and safer is to naturally boost your immune system and/or to take a voluntary vaccine. Note that our immune systems can be temporarily depressed when don’t eat/ diet properly, when we are under mental stress or environmental stressors. Th is the time that common viruses can take root in the body.

    If the virus has a high recovery rate then there will be less emphasis on treatment and more emphasis on preventing spread as the immune system “treats” the body from the virus. You claim that the VAST MAJORITY OF PEOPLE WHO BECOME SEVERELY ILL FROM COVID RECOVER. This is a claim not FULLY supported by data. I am assuming the qualifier of SEVERELY ILL as in needing hospitalization. Unfortunately the odds of dying are greater when persons reach this stage. In this case drugs may have to be resorted to for treatment of the disease but as I said earlier specific medical trials are needed. A greater percentage that become severely ill do not make a full recovery. Those are the ones that develop Long Covid. This is not a unique outcome for persons who had to be hospitalized from respiratory viruses. Those who contract whooping chough, viral bronchitis, influenza and the common cold can develop pneumonia with varying levels of severity and longevity.

    In theory, yes but not in practice. Apart form the basic health issues that can be treated at the polyclinic, once things get more complicated, the cost start to pile up. The doctors at the QEH wil soon tell you that you need this test and that test and unless you want to wait for months, you have to pay for them at private providers. I have plenty of knowledge of this.

    @Crazy John Jacob. If according to your hypothesis, his virus is spread by such a common medium as water then why has the positivity rate remain so low for the duration of this 20 month pandemic. Even with the advent of the Delta variant the positivity rate has not passed the 5-10% rate. Most of the water we use (in comparison to air we breathe) is distributed by an insolated\isolated treated medium ( the treated water supply via pipes ) that is less prone to foreign contamination \ dilution. Virions not exposed to the “environment” have a higher probability of surviving and infecting people at a greater scale.

    The family clusters have a logical explanation. Family members bring COVID-19 into their homes where we do not follow virus prevention protocols. The virus is then spread to other family members. This happens with the common cold and influenza, it happened when was there was an outbreak of gastroenteritis in Barbados approx. a decade ago.

    As you inadvertently brought to light, the confinement to our homes may only be deepening this spread in the family. I think you can join me in the call for ending the curfews and lockdowns!!!!!. I should have the right to party hearty, in an outdoor environment for an extended period or watch sports in an outdoor stadium away from my miserable and now probably infectious relatives and in-laws !!!!!!!!

    For the simple reason that there are several water sources throughout the island and only a couple are impacted.

    The folks who live in the distribution zones fed by these few play Russian roulette every time the take a drink of water or a shower.

  • In Barbados, I can remember only one death where the vaccination status was not divulged at the time of the death announcement.

    All the other deaths that I can remember were of the unvaccinated.

    But people are reaching past the obvious correlation of vaccination:death stats and grabbing stats that bear no correlation.

    If the delta variant is more contagious then the previous variants then there will be more infections and hospitalisations.

    But without the vaccine it would BE WORSE!

    Indeed. But some of us instead of trying to get an understanding of what is actually happening in the real world in the hope that such an understanding can help us work out what to do, just prefer to bury our heads in simplistic narratives and hope that by constantly repeating them, it will make them real. But reality is a stubborn thing. I remember the confident pronouncements from England last year that once 70% of the population were vaccinated, herd immunity would be achieved and both the vaccinated and unvaccinated would be protected after that. Reality hasn’t quite turned out that way.

    In my opinion, our strategy needs to be more targeted and should include the following elements:

    increasing the capacity of the health care system since its inadequate nature is a major contributory factor to the situation we find ourselves in. If your health care system is inadequate for the day to day needs of the population, how can you expect it to cope in an emergency? Hats off to all the health care workers who are doing their best in this situation and I acknowledg tha the government has been making efforts in this direction. We should learn from this experience that a modern, properly funded and well organised health care system that even politicians and their families are comfortable using is not an optional extra in 2021.
    Get our medical researchers to work to help us have more precise ideas of the profiles of those who are infected, need hospitalisation and die. Providing a model that tells us up to 500 people a day will be infected does not help us at all. According to the official statistics to date, we have had over 6000 cases, an unknown number who have required hospitalisation and 53 deaths. Can’t our researchers use this data to tell us the kind of profiles that put individuals in the different categories? We may find that just having co-morbidities is not the whole explanation, but we won’t know if we don’t look.
    Target our approach to people depending on their levels of risk and abandon this idea of vaccinating every Tom, Dick and Harry. For those at low level of risk, encourage them to adopt behaviours that would lessen their chances of becoming infected. The chances are, however, that if they do become infected, they are not likely to die or even need hospitalisation. So we should focus our limited resources on those at high risks. Give them advice on how to protect themselves and this may include prophylaxis and being vaccinated. While some people may not like the mention of ivermectin, there are research studies which suggest that it is associated with lowering the incidence of covid 19 infection https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7698683/
    For those who actually fall sick from Covid, focus our effort on improving treatment and don’t be afraid to try out things that others claim work. After all, we are givng people these experimental vaccines and not making that clear to them. The Astra Zeneca clinical trials don’t finish until 2023. Any experimental treatments should themselves be part of a properly conducted study so that lessons can be drawn from them. Researchers we need you here again. As I have said previously, in China they have used traditional chinese medicine as part of their covid containment strategy. We have good relations with China. Have we asked them what they are doing in this area?

    These are just my initial ideas and I’m not medically trained but I’m hoping that together we can begin to try to work out what to do with the aim of protecting the health and lives of as many of us as possible. Don’t sit around thinking that the government is going to come up with the answer.

    Other interesting directives from A&B’s COVID-19 Management Policy:

    “With effect from 20th September. 2021 Cabinet agreed that all unvaccinated government employees to include the Public Service, statutory corporations and companies in which the government owns the majority shares shall be required to remain at home until proof of vaccination is provided.”

    What Browne has instituted is essentially mandatory vaccination. To be FAIR to the Mottley administration, remember, when rumours of such surfaced in Barbados, it resulted in a protest march, endorsed by the ‘unofficial’ opposition party.

    The usual suspects would laud Browne’s measures and criticize ‘government’ for not taking a similar course of action.
    Could you imagine what would happen in Barbados, if ‘government’ implemented such directives, especially involving the non-payment of non-compliant ‘civil servants?’
