The Covid 19 pandemic rages largely because of the highly infectious delta variant and it has raised questions about the quality of decision making by the public health establishment. 

In Barbados recommendations from the Barbados Association of Medical Practitioners (BAMP) continue to be at odds with public health directives. Acting Chief Medical Officer Dr. Anton Best was obliged to support government’s recent decision to amend the travel protocol to allow fully vaccinated visitors with a valid PCR test to skip a second test and to observe quarantine. Bear in mind the ease in the entry requirement comes at a time the Covid 19 infection curve has not plateaued. 

The other question being asked is the reluctance of the medical fraternity in Barbados, both, public and private to deviate from the ‘script’ by prescribing off-label drugs like Ivermectin and Hydroxychloroquine. Although the establishment has not authorized these drugs to treat Covid 19, several favourable reports have been reported by some medical practitioners. We know the several Covid 19 vaccines are certified to prevent serious illness and death BUT …

Has the time come for Barbados health officials both public and private to agree to expand therapeutic options to treat Barbadians in the prevailing environment, approved by the FDA be damned?

Enclosed are the charts for the week ending 29th October. The charts seem to be suggesting a plateauing of sorts around the mid 300s +ve cases per day, while anecdotal info suggest that this might be an underestimate.  Deaths have been staggering.  We are now at a total of 151 deaths.  Uptake of the vaccines are still trending upwards.  Source: Lyall Small

    What do you see?

    I see two level/flat parts of the graph where there is a slight rise in cases.

    We had one problem in these periods.

    We did nothing to prevent what follows.

    There are two significant rises (circled yellow).

    In October 2020, 17.65 inches of rain fell where I live.

    In June, July and August 2021 real rain fell!!!

    It is obvious from the graph there is some physical kick, impulse if you like, which makes the cases rise.

    All we have to do is to figure out what the impulses are and stop them happening.

    I’s got to be the water.

    Sometime later in 2021 or early in 2022, the rise in cases will plateau at the third red level …. until …. !!!!!

Right away in Genesis 1:2, “The earth was a formless void and darkness covered the face of the deep, while a wind from God swept over the face of the waters.” Water is such an essential component of life, it was created on the very first day.

    Vaccine Booster Jabs

    So what did the studies show?
    It depends on which set of studies you consider. In June, the National Institutes of Health began its own study looking at what happens when people fully vaccinated with Pfizer, Moderna or Johnson & Johnson get a booster of the same vaccine or switch to a new one. The study looked at nine different combinations of vaccines and boosters, with 50 volunteers in each group.

    Early results looked at neutralizing antibodies, which are the specific antibodies that stop the virus and protect you from getting sick. All the booster shots stimulated a neutralizing antibody response, but there were differences. Those who received the Moderna vaccine for their first two doses and Moderna as a booster had the highest antibody levels. Second place went to people who got two doses of Pfizer, followed by Moderna.

    But it’s important to note that the small study groups weren’t designed to compare which shot was best, and the first studies used a full dose (100 micrograms) of Moderna, and not the half dose that has been approved. It’s possible that differences in the study subjects led to the difference in results. And while the difference in antibody levels sounds impressive, it’s probably not all that meaningful in terms of protecting you in the real world.

    The biggest differences in antibody levels were seen in the Johnson & Johnson recipients, who showed a fourfold rise in neutralizing antibodies after the J.&J. booster, but had a 76-fold rise after the Moderna booster and a 35-fold increase after a Pfizer booster.

    Does that mean if I had Johnson & Johnson, I should definitely switch to Moderna or Pfizer?
    Not necessarily. For J.&J. recipients, who initially would have received a single dose, there’s another study to consider. This one included 30,000 people and looked at overall protection from the coronavirus. That study found that a second dose of J.&J., at least two months after the first, resulted in 94 percent protection against mild to severe cases of Covid-19.

    What’s intriguing about the Johnson & Johnson vaccine is that it appears to trigger a different part of the immune system, stimulating not just neutralizing antibodies but also T cells, possibly resulting in more durable protection. The N.I.H. study will eventually look at T-cell response following the various booster shot combinations, but the data aren’t available yet.

    So how should I decide which one to pick?
    All the booster shots stimulate the immune system, so the answer about which shot to get depends on your priorities and personal risk. Here are some examples to help you decide.

    Talk to your doctor: Depending on your personal health circumstances — whether you have underlying health problems, or are prone to blood clots or heart problems, or have been undergoing cancer treatment — your physician might have an opinion about which shot is best for you. Different vaccines, for example, have different possible side effects.

    Convenience: If you just want convenience, pick the shot that’s easiest to get. My 80-year-old mother-in-law, who lives in New Mexico, originally got the Johnson & Johnson shot because that’s what was offered in the small village where she lives. Her plan is to get whatever is offered by her local provider because finding a different shot would require a long drive. My advice to her is to get whatever shot she can as soon as she can. It’s probably going to be a J.&J. booster, which I know will give her more protection than she has now.

    Concerns about risk: People who are particularly anxious about Covid-19 may decide to base decisions about booster shots on preliminary research and pick Moderna, because of the early research showing it stimulates a higher level of neutralizing antibodies.

    Familiarity: Some people may make decisions based on the experience they had with their first shot. They already know their body handled the first dose with no complications, so they may be inclined to pick the same vaccine for the booster shot.

    Dr. Asaf Bitton, executive director of Ariadne Labs at Brigham and Women’s Hospital and the Harvard T.H. Chan School of Public Health, said he’s received a number of questions from patients about which shot to get. For patients who received Johnson & Johnson, he advises them to mix and match with Moderna or Pfizer, based on the preliminary study data showing a higher antibody response. But for patients who have received Moderna or Pfizer, which are mRNA vaccines, he suggests sticking with what you know if you didn’t have any complications with the first two doses.

    “The pragmatic side of me says if you got Pfizer and you did fine with that, then getting a booster of the same one makes sense,” said Dr. Bitton. “To track down that pharmacy that has Moderna instead of Pfizer — is it worth it? I’m not convinced yet with the data we have that it is. Unless you find yourself only able to get one particular kind, I’d say stick with what you’ve got in the mRNA family.”

    Does it matter that the Moderna booster is only half a dose?
    The N.I.H. study of booster shots is looking at whether there’s a difference in response between those who received 100 microgram boosters of Moderna and those who received a 50 microgram dose. Those results aren’t available yet, but it seems unlikely there will be much of a difference, if any, say experts.

    How long does the booster last? Will I need another one soon?
    There’s not an answer to that question yet, but we’ll find out in the coming months as scientists continue to study large groups of people who have been vaccinated and received boosters.

    What a diffference a year makes.

    When we all thought that we need a super medication against COVID, what this country did just dismantle the pandemic worry about.

    The Indonesian Food and Drug Monitoring Agency or BPOM has granted the Emergency Use Authorization for anti-parasitic drug ivermectin on July 15, 2021, as the therapeutic drug to support the Covid-19 treatment.

    The country was at its worst-case scenario epidemic according to its officials as the medical authorities were desperate to control the outbreak and Indonesia has 277 million people and is considered to be the world’s fourth most populous country.

    The Indonesian government on Thursday (July 15) began distributing free medicine and vitamins to self-isolating Covid-19 patients in high-risk areas as the highly transmissible Delta variant of the coronavirus continued to rip through the country, emptying pharmacy shelves.Each package will have seven days worth of therapeutic Covid-19 drugs and vitamins and will be given to asymptomatic patients as well as those with mild to moderate symptoms including fever and dry cough. The medication for the latter group will require consultation with a doctor and a prescription.

    Medical facilities are stretched thin, and demand for oxygen and medication has also soared. As scores of people are isolating themselves at home and self-medicating, prices of drugs have shot up in pharmacies and online. The health ministry has since moved to cap the prices of drugs such as favipiravir, remdesivir, and ivermectin.

    On the other hand, Health Feedback has said:

    Clinical trials didn’t show a clear benefit of ivermectin in reducing COVID-19 severity, hospitalization rate, or mortality. Epidemiological evidence also doesn’t indicate that ivermectin helped curb the number of COVID-19 cases and deaths in Indonesia or other countries. However, these countries implemented other measures that have proven effective in limiting the spread of the virus, including vaccination and restrictions, which likely contributed to the decline in COVID-19 cases.

    Simply because two events coincide in time doesn’t necessarily mean that one caused the other. Firstly, ivermectin endorsement doesn’t tell us whether the Indonesian population used ivermectin more than populations in other countries.

    Secondly, even if they use of ivermectin did increase following the recommendation, we still can’t assess whether the drop in COVID-19 cases in Indonesia was due to the use of ivermectin or to other factors. Epidemiological data from countries that recommended ivermectin, such as India and Peru, doesn’t indicate that promoting the drug reduced the spread of the disease or its death toll. Both countries experienced multiple COVID-19 waves with high mortality throughout 2020 and 2021, and both withdrew ivermectin recommendations in 2021.

    I am not happy about the current death rate among the unvaccinated. We need at least 20 victims a day to get the backward masses running to the vaccination centre.

    If you had done the math on last year’s numbers, 7 deaths, you would find that you were out by a factor of about 20.
    So there should only be 500 to 1,250 cases in existence at the end of last year.
    There were actually 383 cases as of 31st December 2020.
    So far so good …. plausible.”

    You are surprising me with the “actuarial maff” John Boy!!!!! I was here hoping that your mind was slowing returning!!!!. But then you proceeded to state

    However, Dominica had by then found 88 cases when the logic suggests they should have had ZERO!!”

    Assuming that those DETECTED cases were ALL the ACTUAL CASES, deducing the deaths from the cases (using the 0.3% – 0.6% global case fatality rate) reveals 0.264 – 0.528 deaths. i.e. effectively there were 0 Covid-19 deaths since you can’t have 1/2 or 1/5 of a person being dead (well in your case you are partly brain dead so I guess it is possible to be part dead….. lol)

    You also stated:-

    For Barbados this now exponential surge in cases happened from late August- early September. Using your “change in transmission posit” I am assuming either at that time or a few months before:- .

    1) Barbados experienced an exponential increase in water. In terms of amounts, there would had to be a catastrophic rainfall even or daily rainfall would had to significantly risen in the months before all through now OR

    4) As 555dubstreet alluded to, drinking water would have now surpassed air as the better medium for the spread of a RESPIRATORY VIRUS

    My counter (and might I say more rational) hypothesis to yours John boy was that the virions were vexxed that they couldn’t celebrate Crop Over for the second straight year in a row so they are doing dixie\ going mad on the Bajan populace. LOL

    I will go with “real science” that a more contagious mutation of the virus entered or evolved around or just before that time in Barbados which coincidently happened to have the same genetic structure of the Delta Variant that appeared globally

    Here is how I read Indonesia.

    A slow rise to 200K cases followed by a slow fall yo very few cases.

    I disregard the surge/spike because that followed floods and floods are an act of God.

    However, it is now experiencing once more floods again as of early October.
