The fight against HIV and AIDS continues

PORT OF SPAIN, 30 May, 2014โ€”In a new move to jointly address the HIV epidemic and improve the lives of people living with the virus, countries in Latin America and the Caribbean and partner organisations have established new targets for expanding diagnosis and antiretroviral treatment (ART) and reducing patientsโ€™ viral loads by the year 2020.

The new targetsโ€“dubbed โ€œ90-90-90โ€โ€“were adopted during the First Latin American and Caribbean Forum on the HIV Continuum of Care, which is being held this week in Mexico City. The Forum was organised by a coalition of partners including Mexicoโ€™s Secretariat of Health, the Joint United Nations Programme on HIV and ADS (UNAIDS) and the Pan American Health Organisation/World Health Organisation (PAHO/WHO).

โ€œExpanding early diagnosis and treatment combines the clinical benefits of early treatment for patients with benefits to the population of preventing transmission,โ€ said Cรฉsar Nuรฑez, Regional Director of UNAIDS for Latin America.

Massino Ghidinelli, chief of PAHO/WHOโ€™s HIV, Hepatitis, Tuberculosis and Sexually Transmitted Infections Unit, said the new targets are a step forward in controlling the HIV/AIDS epidemic. โ€œIf we want more people to be on treatment and to achieve undetectable viral loads, they have to know their diagnosis and begin treatment early.โ€

In 2012, some 1.8 million people in Latin America and the Caribbean were living with HIV, and some 98,000 people became newly infected, according to UNAIDS estimates.

The new โ€œ90-90-90โ€ targets

Target 1: Increase the proportion of people with HIV who know their diagnosis to 90%

New PAHO/WHO estimates for 2013 suggest that 70% of people living with HIV in Latin America and the Caribbean know they are infected. This is an average based on data from countries that account for 62% of the regionโ€™s HIV epidemic. However, in some countries fewer than half know their HIV diagnosis. Expanding testingโ€”by increasing the availability of tests and involving communities and civil society in the effortโ€”will lead to more people with HIV seeking the treatment they need.

Target 2: Increase the proportion of people receiving antiretroviral treatment to 90%

Some 725,000 people with HIV were receiving antiretroviral treatment in Latin America and the Caribbean as of December 2012, and preliminary PAHO/WHO estimates suggest this number increased to more than 800,000 by December 2013.

Treatment coverage rates in Latin America and the Caribbean are higher than in any other low- and middle-income region. However, WHOโ€™s newest HIV guidelines recommend earlier initiation of treatment (when oneโ€™s CD4 cell count falls to 500 cells/mmยณ or less), which has increased the number of people who meet the criteria for receiving treatment. Applying the new criteria to 2012 data would reduce the regionโ€™s coverage rate to 43%.

Expanding treatment would contribute to better health for people with HIV, reduce cases of AIDS and prevent new infections. Toward this end, countries agreed to revise their models of care to make treatment more accessible.

Target 3: Increase the proportion of people under treatment who have an undetectable viral load

PAHO/WHO data indicate that some 66% of people with HIV in Latin America and the Caribbean had suppressed viral loads in 2013. Reaching the 90% target will require improved patient adherence to treatment.

Suppressed viral load is critical for controlling the harmful effects of HIV infection on peopleโ€™s health and also significantly reduces the risk of infecting others.

To promote adherence and keep patients under treatment, HIV health care must be decentralised to levels that allow greater interaction with the community.

Countries also agreed on a fourth target: reducing delayed diagnosis. As of 2013, half of countries providing data reported that at least a third of people with HIV were at an advanced stage of immunological disease at the time of their diagnosis. This situation has been improving, however, with late diagnoses declining from 40% to 35% between 2012 and 2013, reflecting expanded HIV testing in a number of countries.

In addition to PAHO/WHO, UNAIDS and Mexicoโ€™s Secretariat of Health, the Latin American and Caribbean Forum on the HIV Continuum of Care also received support from the Government of Brazil, the United States President’s Emergency Plan for AIDS Relief (PEPFAR), the AIDS Healthcare Foundation (AHF), the Horizontal Technical Cooperation Group (GCTH), the International Association of Providers of AIDS Care (IAPAC), the U.S. Centers for Disease Control and Prevention (CDC), and the Pan Caribbean Partnership against HIV and AIDS (PANCAP).

    This is one of the best pieces I have read on this issue by a very brilliant scholar.

    In capitulating to the pro-gay interest groups, the University of the West Indies โ€” we have no doubt โ€” would have considered the various agendas of its donors, many of whom have serious financial clout and are very favourable to the gay community. But the decision has left the university on the horns of a dilemma.

    Must all its officers now subscribe wholly to the lifestyle of the various communities they serve? Was it in Dr Bainโ€™s job description, as well as that of the other academics at the UWI, that he must hold no public opinion against homosexuality? How far must the university go in censoring and muzzling its academics to suit interest groups? Is this the end of scientific freedom and freedom of expression in the academic community?

    Of course, the decision would also have confirmed what many people have long held that homosexuals have strong influence in the running of the university and have firmly ensconced themselves in the human rights lobby.

    We have been consistently circumspect in this editorial space where the gay lifestyle is concerned, promoting tolerance about peopleโ€™s right to their lifestyle if it does no harm to other citizens. But we admit to growing unease that the gay community is not as tolerant as they want others to be of them.

    If action speaks louder than words, Dr Bainโ€™s hard work to better the life of HIV/AIDS sufferers should have outweighed his personal opinion against homosexuality. A surgeon may be publicly against murder but that should not stop him from saving a murdererโ€™s life in his professional capacity. And who is to say that all HIV/AIDS sufferers are men who have sex with men (MSMs)? Who is speaking for those who are not gay?

    We should be free to admit that not all MSMs are promiscuous, wild and careless about sex. But there are some who are, and this makes Dr Bainโ€™s pronouncement about the spread of HIV/AIDS among MSMs something to be taken seriously.

    Discriminating against Brendan Bain
    Story Updated: May 31, 2014

    The dismissal of Prof Brendan Bain as director of the Caribbean HIV/AIDS Regional Training (CHART) Network has continued to trigger debate, with each faction pushing their position. But then, that is what freedom of speech means.

    Commentators like myself have our own personal positions, but that does not stop us from pointing out inconsistencies and blatant falsehoods, no matter who utters them. Let us get one thing clear at the start: HIV/AIDS is not a homosexual disease; heterosexual persons get it, and people can also contract the disease outside of sexual intercourse (eg, when infected drug users share intravenous needles).

    But it seems that the LGBT (lesbian, gay, bisexual, transgender) lobby believes that HIV/AIDS is an LGBT disease, for they have captured the UWI-run CHART project, seeking to turn it to serve their own political ends. Isnโ€™t it ironic that the LGBT lobby is objecting to Prof Bain stating that homosexual men are most likely to contract HIV/AIDS, while seeking to turn this health-care project into a gay-liberation project?

    If homosexuals are no more likely to get HIV/AIDS than anyone else, why would it matter whether buggery is criminalised or not? It is because they know that Prof Bain is right, and that homosexual men are more likely to contract HIV/AIDS than anyone else that they want to capture this UWI-administered prevention-and-treatment project.

    In Thursdayโ€™s Gleaner, Prof Rose-Marie Belle Antoine, dean of the Faculty of Law, UWI, St Augustine, accuses her former colleague of supporting the โ€œretention of a discriminatory regime that excludes persons from enjoying rights of equality on the basis of their sexual orientationโ€.

    One must presume the law professor knows the law. Pray tell, professor, which Belizean law or Jamaican law conveys to LGBT people โ€œrights of equality on the basis of their sexual orientationโ€? This is precisely what the Belizean court case in which Prof Bain testified is seeking to determine, and precisely what the case involving Javed Jaghai and the attorney general of Jamaica is seeking to determine. It seems that the good professor is prejudging the outcome of these cases.

    Neither the United Nations Universal Declaration of Human Rights nor the Bill of Rights in the Jamaican Constitution admits โ€œrights of equality on the basis of … sexual orientationโ€, and so Prof Bain cannot be successfully accused of attacking rights that do not exist. She and the gay lobby may wish these rights to exist, but they donโ€™t, and it is disingenuous and misleading to speak as if they do.

    Discrimination is an emotionally charged word, but the good law professor will also know that not all discrimination is contrary to law. The fire service may discriminate against handicapped persons in their hiring practices. The military may discriminate against obese persons in their recruiting exercises. The Constitution of Jamaica discriminates against convicted criminals, bankrupt persons and foreign nationals running for public office. The church will legitimately choose not to hire satanists and paedophiles in its schools. I certainly am quite discriminating in the restaurants I choose to eat in.

    In these days, when the importance of uplifting values and attitudes is recognised, some may feel the need to choose carefully the persons hired as household workers and school workers. The LGBT lobbies have chosen to discriminate against Prof Bain, declaring him unsuitable to be director of the CHART programme because of his personal values. Even the LGBT groups support discrimination; they just donโ€™t want anyone to discriminate against them, however legitimately.

    There is no need for a project like CHART, dedicated to โ€œprovide access to quality HIV and AIDS prevention, care, treatment, and support services for all Caribbean peopleโ€, to have as one of its goals the removal of buggery laws across the Caribbean.

    I challenge the LGBT lobby to produce the evidence that there are persons with HIV/AIDS who cannot access treatment because of buggery being illegal. No one has to say how they came to be infected.

    Demanding that the buggery laws be repealed because they prevent people from receiving medical treatment is a convenient ruse to get the gay lifestyle normalised. We must not fall for this samfie trick. โ€”

    I am quite sure of Dr. King was living today, he would have fought against two consenting adults of the same gender righy to choose their own course in this life. lol I haven’t any doubt that had Dr. King been living today, he would have spoken out against the biblical immorality that is associated with the gay and lesbian lifestyle. But he wouldn’t have gone as far to say that two consenting adults of the same gender, couldn’t choose to do with their lives as their please. Because he would have known this wasn’t his place as a man of God.

    A case of reverse discrimination

    The University of the West Indies (UWI) should reinstate Professor Brendan Bain, poste haste, as Director of the Regional Co-ordinating Unit of the Caribbean HIV/AIDS Regional Training (CHART) Network because he was not accused, nor was it revealed that he had been derelict in his duties in that role.

    So the suggestion in another section of the press by Dr. Carolyn Gomes, executive director of Caribbean Vulnerable Communities Coalition (CVC), and its finance manager, Ivan Cruickshank, that it was a conflict of interest issue with the Professor, vis-a-vis his association with CHART and then giving his professional opinion on the September 10, Caleb Orozco constitutional challenge against the Belizian country’s criminal code to decriminalise buggery, was only a lame excuse for CVC to acquiesce to the demands of the 33 lobbying groups who had advocated for Professor Bainโ€™s removal.

    The brouhaha had resulted because Caleb Orozco, a gay Belizian man was arguing that the ten year prison sentence for buggery violated his right to human dignity and personal privacy. Professor Bain, a pioneer in clinical and infectious disease practice, had testified that the risk of contracting HIV was significantly higher among men who had sex with other men.

    According to an article in the Jamaica Observer by Karyl Walker (Protesters picket UWI in support of sacked professor, May 22, 2014), a group of concerned citizens gathered in front of the UWI main campus gate to lend their support to the sacked professor. And at the time of writing hundreds had signed a petition in favour of Professor Bainโ€™s reinstatement. More healthy protests are needed to avert this reverse discrimination,
    instigated by the 33 coalition groups that were apparently instrumental in the professor’s ouster.

    Does any lobby group have the right to stifle an individual’s professional opinion because it runs counter to their specific agenda? Freedom of speech should be for all and a professional opinion should be examined for its content and not be discounted and replaced with emotional feelings.

    Historically, universities have been the seat of learning and free thinking. The administrators should not now join forces to be part of a growing trend to replace fact for fiction and appease certain groups to remain politically correct. As the battle lines are drawn, it was reported in an article published in The Jamaica Gleaner by Senior Reporter Erica Virtue (Battle over Bain continues on May 25, 2014), that CVCโ€™s Executive Director Dr. Gomes remains adamant that UWI will not reverse its decision and it is futile for the protests to continue.

    However, in the same article, it was also reported that the protesters were not daunted and that they had started an email campaign and had also gathered about 2000 individuals to write letters, for Professor Bainโ€™s reinstatement, to the Vice-Chancellor of UWI, Professor E. Nigel Harris.

    Letโ€™s hope that common sense prevails and Professor Brendan Bain would be judged by his stewardship at CHART.     A case of reverse discrimination

    The University of the West Indies (UWI) should reinstate Professor Brendan Bain, poste haste, as Director of the Regional Co-ordinating Unit of the Caribbean HIV/AIDS Regional Training (CHART) Network because he was not accused, nor was it revealed that he had been derelict in his duties in that role.

    So the suggestion in another section of the press by Dr. Carolyn Gomes, executive director of Caribbean Vulnerable Communities Coalition (CVC), and its finance manager, Ivan Cruickshank, that it was a conflict of interest issue with the Professor, vis-a-vis his association with CHART and then giving his professional opinion on the September 10, Caleb Orozco constitutional challenge against the Belizian country’s criminal code to decriminalise buggery, was only a lame excuse for CVC to acquiesce to the demands of the 33 lobbying groups who had advocated for Professor Bainโ€™s removal.

    The brouhaha had resulted because Caleb Orozco, a gay Belizian man was arguing that the ten year prison sentence for buggery violated his right to human dignity and personal privacy. Professor Bain, a pioneer in clinical and infectious disease practice, had testified that the risk of contracting HIV was significantly higher among men who had sex with other men.

    According to an article in the Jamaica Observer by Karyl Walker (Protesters picket UWI in support of sacked professor, May 22, 2014), a group of concerned citizens gathered in front of the UWI main campus gate to lend their support to the sacked professor. And at the time of writing hundreds had signed a petition in favour of Professor Bainโ€™s reinstatement. More healthy protests are needed to avert this reverse discrimination,
    instigated by the 33 coalition groups that were apparently instrumental in the professor’s ouster.

    Does any lobby group have the right to stifle an individual’s professional opinion because it runs counter to their specific agenda? Freedom of speech should be for all and a professional opinion should be examined for its content and not be discounted and replaced with emotional feelings.

    Historically, universities have been the seat of learning and free thinking. The administrators should not now join forces to be part of a growing trend to replace fact for fiction and appease certain groups to remain politically correct. As the battle lines are drawn, it was reported in an article published in The Jamaica Gleaner by Senior Reporter Erica Virtue (Battle over Bain continues on May 25, 2014), that CVCโ€™s Executive Director Dr. Gomes remains adamant that UWI will not reverse its decision and it is futile for the protests to continue.

    However, in the same article, it was also reported that the protesters were not daunted and that they had started an email campaign and had also gathered about 2000 individuals to write letters, for Professor Bainโ€™s reinstatement, to the Vice-Chancellor of UWI, Professor E. Nigel Harris.

    Letโ€™s hope that common sense prevails and Professor Brendan Bain would be judged by his stewardship at CHART. Also, his professional opinion would have been based on statistical and empirical data. He should not have been summarily dismissed for giving his professional opinion in the Caleb Orozco September 10 Belizian constitutional challenge.

    I think that they call people like me POST-SEXUALS and I hope that post-sexuality is not a Biblical abomination, but if it is and if I will be cast out of the kingdom I am sure that God’s messenger Georgie Porgie will soon tell me.

    So the best we can do is to ask people to take their time getting to know potential new partners, to have HIV tests so that each person knows their status, for those who are HIV positive to take their medicine in full and on time, for those who are HIV positive and who still wish to have sex to take their medicine, to inform their partner before sexual activity and to always use condoms. And to encourage everybody to form faithful live long sexual/emotional/legal/financial unions.

    Because sensible human people, including sensible physicians understand that lifelong celibacy is IMPOSSIBLE for most people.

    Celibacy is especially difficult between late adolecence and early middle age.

    In old age, our diabetes, hypertension and benign prostate hyperplasia helps us to fairly easily manage celibacy LOL!!!!!!!!

    But we must never forget what it felt like to be young with raging, raging hormones. LOL!!!!!!!!!!

    And we must NEVER, NEVER be cruel to the younger generations.

    You’re quite right Simon. But speech which falls outside the periphery of constitutional protection isn’t protected by the constitution. For example: you cannot take up the phone and call in a bomb threat to police. But yet one is exercising his constitutional right to free speech. And this you ought to also remember: one constitutional right to free speech takes a backseat in the interest of national security. There is also a Sedition Act which curtailed free speech during the Second World War.

  50. church law and govt law cannot not operate as equals even both are formed on the foundation of moral values with similar goals in mind based on the premise of upholding that which is right ,,however they both are separate and independent of each other…one based on the teachings of strict religious principles and doctrines and the other based even though inclusive and tolerant of religiosity doctrine have the burden of providing laws that are fair and equitable,,,,, the church for example had remained adamant many years in the principle belief that man is the head of the household,,but as society changed even such principles had to be abandoned reluctantly by the church in order not to be in conflict with govt law and society needs,,,govt role at those juncture was to interject and address and respect the needs of society,,,and the church oblige.,,, .,,the same process has also occurred in the church when at one time religious doctrine and law prohibited woman from becoming priest but now that too has changed … the church no matter how adamant and strong( will) it has been on many issues had to bend their own rules and adjust to govt laws and standards while adjusting to society calls for change,, the same will happen with the issue or issues surrounding gay rights and privileges ..no matter how hard the church battle with the world the world would always come out ahead because in life change is necessary and deeply rooted in change is a call for fairness and justice,,,,,and on this one Basic principle which is also part of bible principles the church cannot stand win or stand alone ,,as the church would be seen as an ostrich with a head buried deep in the sand,,,and having to live in a world unto itself of isolation.., ,,in order for the church to win it has to be flexible and less stubborn ,,ever so often reaching out to those who are seen as outcast. while becoming beacons of hope for righteousness and not self righteousness
