    Symptoms of Multiple Endocrine Neoplasia

    According to the National Cancer Institute, around 1 in 30,000 people are affected with each of the two major types of multiple endocrine neoplasia.

    Multiple Endocrine Neoplasia Type 1
    MEN1 tumors may result in the overproduction of hormones in your body. When specific tumors form in specific endocrine glands, those glands often make too much hormones, which may cause the following:
    • Parathyroid gland – hyperparathyroidism and hypercalcemia
    • Pituitary – oligomenorrhea/amenorrhea and galactorrhea (females) or sexual dysfunction (males)
    • Gastrointestinal tract
    o Gastrinoma – Zollinger-Ellison syndrome (upper abdominal pain, diarrhea, esophageal reflux, acid-peptic ulcers)
    o Insulinoma – hypoglycemia
    o Glucagonoma – hyperglycemia, anorexia, glossitis, anemia, diarrhea, venous thrombosis, skin rash
    o VIPoma – watery diarrhea, hypokalemia, achlorhydria syndrome (lack of hydrochloric acid in the stomach)
    • Carcinoid
    o Diarrhea
    o Facial flushing
    o Wheezing
    • Adrenocortical – hypercortisolism (Cushing’s syndrome due to adrenal tumor) or hyperaldosteronism

    MEN1 also may cause the following non-endocrine tumors:
    • Facial angiofibromas – benign blood vessel/connective tissue tumor
    • Collagenomas – cutaneous nodule
    • Lipomas – benign fatty tissue tumor
    • Meningiomas – benign brain tumor
    • Ependymomas – malignant tumor of the central nervous system
    • Leiomyomas – benign smooth muscle tumor

    Multiple Endocrine Neoplasia Type 2
    In most cases, patients with MEN2 develop medullary thyroid carcinoma (MTC). Signs or symptoms of MTC include:
    • Mass or lump in the neck
    • Goiter (enlarged thyroid)
    • Diarrhea
    Around half of patients with MEN2A and MEN2B develop pheochromocytomas; about a third develop primary hyperparathyroidism.
    A subset of patients with MEN2A may also develop:
    • Hirschsprung disease
    • Pruritic cutaneous lichen amyloidosis
    In many cases, patients with MEN2B will develop the following symptoms:
    • Neuromas and/or ganglioneuromatosis of gastric system
    • Reduced upper/lower body ratio
    • Marfanoid habitus
    • Enlarged lips/bumpy tongue
    • Skeletal abnormalities
    • Joint laxity

  15. What is the New York State Department of Health’s New Guideline of PERC in Air?

    After consideration of the potential health effects of PERC, background levels of PERC in air, and analytical techniques (the ability and reliability of methods to measure PERC in air), NYSDOH recommends that the average air level not exceed 30 mcg/m3. This determination considered continuous, lifetime exposure and sensitive people. Three other ways of expressing the new guideline are 0.03 milligrams per cubic meter of air (0.03 mg/m3), 4.4 parts per billion (ppb) or 0.0044 parts per million (ppm). This replaces the old guideline of 100 mcg/m3.

    An air guideline of 30 mcg/m3 is below the PERC air levels known to cause noncancer effects, including developmental and reproductive effects, in humans and animals, and should be protective against those effects. It is lower than the USEPA’s (2012) reference concentration (RfC)4 for PERC of 40 mcg/m3. The estimated excess cancer risk associated with lifetime, continuous exposure to 30 mcg/m3 is about one-in-one-hundred thousand. Decisions about whether to take actions to further reduce exposure are generally made on a case-by-case basis at this level of risk.

    However, NYSDOH recommends that reasonable and practical actions should be taken to reduce PERC exposure whenever air levels are above background. The purpose of the guideline is to help guide decisions about the urgency of the actions to reduce PERC exposure. The urgency to initiate these actions and to determine, in a timely manner, whether they have reduced exposure, increases with indoor air levels, particularly when air levels are above the guideline.

    Indoor air levels substantially above the guideline indicate a significant PERC source and may require more immediate remedial action. NYSDOH has concerns about lengthy exposure (months to years) to air levels higher than 300 mcg/m3 because the results of a recent NYSDOH study suggested that indoor air PERC levels in apartments (median value of 340 mcg/m3) may have subtle effects on the nervous system (vision function ) of children (NYSDOH, 2010). Thus, NYSDOH recommends taking immediate and effective action to reduce exposure when an air level is equal to or above 300 mcg/m3. In all cases, the specific corrective actions to be taken depend on a case-by-case evaluation of the situation. The goal of the recommended actions is to reduce PERC levels in indoor air to as close to background as practical.

    Why Did the Health Department Reduce the Guideline for PERC in Air from 100 mcg/m3 to 30 mcg/m3?

    The guideline of 100 mcg/m3 was issued in 1997 and was based on the toxicological data available at the time. Since then, many new toxicity studies have been published and the USEPA has completed a comprehensive, state-of-the-science, peer-reviewed risk assessment of PERC. Based on the risk assessment, the USEPA recommended values for evaluating the potential for noncancer and cancer effects from exposure to PERC in air [a RfC (40 mcg/m3) and an air level (4 mcg/m3) associated with an estimated excess cancer risk of one-in-one million, assuming continuous, lifetime exposure]. NYSDOH staff reviewed the USEPA risk assessment and determined that the recommended values are scientifically robust and should replace the values derived in 1997. The USEPA publication of its RfC (40 mcg/m3) necessitated a re-evaluation of the health-protectiveness of the old NYSDOH guideline (100 mcg/m3) because it has been the past practice of NYSDOH to set guidelines at air levels that are equal to or less than a RfC. Consequently, the guideline was reduced to 30 mcg/m3 after consideration of new toxicity data (e.g., NYSDOH, 2010) and the USEPA risk assessment.


  16. Is There a Test to Determine Whether I Have Been Exposed to PERC?

    PERC levels can be measured in the breath for weeks following a high exposure to PERC because it is stored in body fat and is slowly released into the bloodstream and then exhaled in the breath. PERC can be measured in blood. Also, breakdown products of PERC can be detected in the blood and urine for several days after exposure to PERC. Because exposure to other chemicals can produce the same breakdown products in the urine and blood as PERC, the tests for breakdown products cannot determine if you have been exposed only to PERC. Although the tests can show if PERC levels in the body are elevated compared to background levels, they cannot conclusively determine when and for how long a person was exposed, what the source of that exposure was, or whether or not the person will develop adverse health effects.


  17. Has the federal government made recommendations to protect human health?
    The EPA maximum contaminant level for the amount of tetrachloroethylene that can be in drinking water is 0.005 milligrams tetrachloroethylene per liter of water (0.005 mg/L).

    The Occupational Safety and Health Administration (OSHA) has set a limit of 100 ppm for an 8-hour workday over a 40-hour workweek.

    The National Institute for Occupational Safety and Health (NIOSH) recommends that tetrachloroethylene be handled as a potential carcinogen and recommends that levels in workplace air should be as low as possible.

    It seems that the carcinogen PERC has been involved in the cause of the diseases afflicting the ex Louis Lync students.

    The fact that not all the students are afflicted might probably be explained by Knudson’s two-hit” hypothesis of oncogenesis which seeks to explain the inherited and sporadic occurrence of an apparently identical tumor

    Knudson’s theory was developed with respect to retinoblastomas
    Approximately 60% of retinoblastomas are sporadic, and the remaining 40% are familial, with the predisposition to develop the tumor being transmitted as an autosomal dominant trait.

    To explain the familial and sporadic occurrence of an apparently identical tumor, Knudson proposed his now famous “two hit” hypothesis of oncogenesis.

    He suggested that in hereditary cases, one genetic change (“first hit”) is inherited from an affected parent and is therefore present in all somatic cells of the body, whereas the second mutation (“second hit”) occurs in one of the many retinal cells (which already carry the first mutation).

    In sporadic cases, however, both mutations (hits) occur somatically within a single retinal cell, whose progeny then form the tumor. Knudson’s hypothesis has been amply substantiated by cytogenetic and molecular studies and can now be formulated in more precise terms.

    What seems to be sporadic cases of MENS in the past students may be due to the PERC causing two mutations occuring somatically within a single cell followed by clonal expansion of double-mutated cell – tumor.

    Some of the cases might be due to a genetic change (“first hit”) inherited from an affected parent which is therefore present in all somatic cells of the body, followed by a second mutation (“second hit”) caused by the PERC occurring in one of the cells (already hit by the first mutation). cell followed by clonal expansion of double-mutated cell – tumor.

    The members of the families only had to have a predisposition to the diseases – the first hit. They did not have to be exposed to PERC The second hit would be the exposure to PERC

    It is now accepted that there is a genetic involvement in many diseases.

    Environmental exposure to PERC may be the factor that cause the mutationn(s) for the oncogenesis, but there may also be a genetic preposition to cancer in some of those affected..

    Please kindly note that I am not guided by the responses on BU about medical matters. Many untrained people pontificate on NU illogically or emotionally and without a “smidgen” of relevant information.

    What is noteworthy of MENS is the several modes of presentation. This might be most distressing to the patients/sufferers, as well as make it very difficult for the doctors to make a diagnosis, since the patients are probably not presenting simply as is usually the case with the usual run of the mill common disease patterns.

    This .situation ought to be seriously investigated. Would probably be good for a doctoral thesis in Public Health. .

    • MEN I is a disease of 3 P’s (pituitary, parathyroid, and pancreas) plus adrenal cortex
    • MEN II is a disease of 2 C’s(carcinoma of thyroid and catacholamines [pheochromocytoma]) plus parathyroid for MEN IIa or mucocutaneous neuromas for MEN IIB(aka MEN III)

    I am not trying to discourage your efforts to enlighten the BU bloggers. But any good doctor would simplify the unfamiliar medical terminologies, so that the practical man like myself, who has little or no familiarity of them because he hasn’t studied them with a focused effort, as you have done. Now, I visit several doctors offices during week because my job requires it, and from time to time during those visits, I would engage some of these doctors in conversation; on issues relating to drug – therapy as well as behavioural- therapy etc. And the majority of these doctors would simplify their conversation to accommodate or compensate for my lack of knowledge regarding the medical jargon. In essence, it is kinda like a lawyer trying to explain the legal vernacular to a doctor.

    i was involved in the Future Centre Trust at its inception, and haVe a nephew and niece who went to this school.

    THE FTC needs to acquire the student and staff rolls for the duration of the schools existence, and make calls for all of these folk to contact them to report on the length of time they spent at the school and about their health status, and the nature of their disease if they have any. This would include their symptoms and diagnoses.

    For those who have cancers perhaps following trends of cancer in their families could be compared.

  31. its the asbestos that gave people cancer and other diseases, Barbados was suppose to stop using that product a long time ago, but they wanted to buy cheap materials from china and other countries, that don’t investigate their materials. Almost all the older government houses are built from asbestos, don’t use it its made mud, dirt or something poisonous.

  33. some people have very strong immune systems, so just because the watchman lived a long life, don’t mean everyone will, some people had cancers in the face, people would just laugh at them, but its the building supplies the country import, bajans really have to wake up and ask questions about building supplies, their water and other matters, because buying cheap down graded materials, will hurt lots of people in the long run, too bad the long gone deceased people wont get any compensation,

  35. no its not George, its a combination of lots of material that was made with the asbestos material, when the buildings fell in NY during 911, lots of people got respiratory problems, that’s because material in the air was bad, lots of people died because of breathing in those bad products in the air, so again its a combination of the products being made in the asbestos.

    The work you are proposing that the FTC do may be beyond them. They will need to have the services of a medical epidemiologist. They will also need the specialists in the area to review and concur. The CDRC may be a good unit from UWI to get involve. This will involve the review especially of medical notes over at least a twenty year period. You can not leave out the ones former students who appear to be well!!

    I knew many people who have attended Roebuck Secondary and have many personal friends who are now in their middle to late forties, who have also attended that school and they’re in perfect health as I speak. During my years at Roebuck Boys Primary, I visited Roebuck Secondary to see my friends there countless number of times, and I am still in good health as we speak, thanks to God.

    I am not advancing the view that there wasn’t some very wrong with that school and persons who alleged that their had gotten sick there. But to make a convincing case/ argument, one obviously must and ought to also look at those people who were in that same environment and are now in good health, as we seek to ascertain the etiology of this usual medical phenomenon.
